1/28/2024 EnchantaVaughn Approved QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a relaxed mood evident by her facial expressions such as a smile on the face and engaging in appropriate conversation with QMHPA. QMHPA inquired about her day. Kaila did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Kailaâs current thoughts, feelings and emotions. QMHPA engaged Kaila in a discussion surrounding her medication management. QMHPA assisted Kaila with identifying appropriate calming skills to manage depression and anxiety. QMHPA engaged Kaila in a skills building activity. QMHPA informed Kaila that they purpose of this activity was to educate her on the signs and symptoms of depression/anxiety. QMHPA implemented Kaila endure in enjoyable activities to avoid depression. QMHPA engaged Kaila in another skills building activity. QMHPA explained to Kaila that the purpose of this activity was to demonstrate different coping techniques on managing depression. QMHPA assisted Kaila with identifying positive moods. QMHPA assisted Kaila with recognizing her triggers and how to redirect them when they occur. QMHPA processed with Kaila being able to think of an appropriate and inappropriate way to react to her triggers by choosing the best reaction. Kaila appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Kaila verbalized to QMHPA that she has been taking her medication but it has not been like she should be. She indicated to QMHPA that she doesnât like some of the side effects (makes her to sleepy). Kaila was agreeable with appropriate calming skills. During the skills building activity, Kaila was able to identify signs of depression and admitted to being depressed at times. Kaila was able to identify enjoyable activities but not within reason. Kaila stated she can recognize her triggers but she doesnât control them. Kaila identified some of her triggers being her family members, her boyfriend, and her family members. She also indicated to QMHPA that one of her triggers is when she donât have money to pay all her bills the way she needs to and she then has to figure it out by herself.
